FSMB launches new identity verification procedure

The Federation of State Medical Boards (FSMB) has announced that its Federation Credentials Verification Service (FCVS) will implement a new procedure for identity verification. Effective February 6, each applicant will be required to complete and mail a Certification of Identification (CID) form, with the exception of those designating to Oregon Medical Board or Vermont Board of Medical Practice. The CID form requires notarization and attachment of a photocopy of an applicant’s birth certificate or a photocopy of his or her passport. The CID form and instructions are available for download beginning today.
