Medical staff, leadership have key roles in Connecticut's high-reliability initiative

Loeb, PhD, executive vice president for healthcare quality evaluation at The Joint Commission, urge hospitals to adapt lessons from high-risk industries and make the changes necessary to achieve the goal of zero patient harm.
