Tip of the week: Identify which individuals may submit references for new applicants (free form)

In the past, institutions often relied solely on references supplied by the applicant. But now it is recommended, and in some cases required, that the institution identify which individuals may submit references. Try the following policy on for size:

It is the policy of this institution to process applications for appointment and/or clinical privileges only after receipt of acceptable clinical references (either completed questionnaire or other) by the institution.  Such references must be provided by individuals knowledgeable about the applicant’s past clinical and professional activities. The application must include references from the applicant’s immediate past or current practice settings (such as residency, current hospital, managed care organization, or immediate past hospital or managed care hospitalization). This institution requires references from at least three of the individuals designated in this table.

In the event this institution does not receive references from the designated individuals, processing of the application will cease, and the medical staff services director and credentials chair will review the application to determine further action.
