Intended practice plan for advanced practice professionals
One way to determine future plans for low-volume practitioners is to ask them to complete an intended practice plan. This can be used for new applicants as well as at the time of reappointment. Utilization of this form is a quick way to establish the advanced practice practitioners' (APP) practice plans. This form can be used for all new APP applicants as well as for low/no-volume practitioners.
