Allied health peer review case rating form: Nurse practitioner

This tool from HighPoint Health System in Gallatin, Tennessee, is a peer review rating form allowing reviewers to identify core privileges and special procedures for the given specialty that may require additional training and certification. While this form is specifically designed for nurse practitioners, with its accessible formatting and emphasis on scope of practice, it can be easily adapted for other APP disciplines.
