A key part to any credentialing application is the ­attestation, acknowledgment, and release forms. These documents protect your organization as well as the entities that release information to your organization throughout the credentialing process.

The length and format of attestations, acknowledgments, and release forms vary among organizations. Some are several pages long while others are just summaries. Either format is acceptable as long as several key elements are included as well as any state- or organization-specific requirements. Legal counsel should always review and approve any substantive changes to the information included in these documents.
