FPPE monitoring form

Proctors may not know what information they need to collect or report. To help proctors succeed, give them a standardized form to complete when observing another physician. Today’s free resource is a summary focused professional practice evaluation (FPPE) monitoring form. This form is completed by the proctor and outlines the type of FPPE conducted (chart review, external peer review, direct observation, etc.) It also lets the proctor state his or her opinion on whether FPPE needs to be continued, whether the physician under review is performing well, or whether immediate patient risk exists. The summary FPPE form is then sent to the credentials committee for review.
