Eliminate "other" on privilege forms
Communication with new applicants regarding their request for clinical privileges will help clarify what may be required on the part of the applicant (e.g., additional documentation specific to certain privileges and what that documentation needs to be). It is also important to be sure that someone is not requesting a privilege/procedure that is currently not offered. This can be avoided by eliminating an “other” option on privileges forms. Include a statement on the privilege form that if the applicant is requesting something not currently on the form, it should be a separate request which will then be evaluated according to the policy for reviewing new requests. These requests may be based on new technology/innovations that have not yet been considered by the organization or one that the organization is in the process of evaluating.
