Brigham and Women’s reports errors to staff in drive for improvement

Brigham and Women’s Hospital in Boston distributes an online publication to its 16,000 member staff, disclosing errors and mistakes and the lessons learned from them. The aim is to encourage staff to talk openly about their mistakes and propose solutions, and help make sure errors are not repeated.
