Try a prospective approach to proctoring

Prospective proctoring is a powerful and greatly underutilized tool. The approach involves a one-on-one discussion with the practitioner of concern to learn how he or she will tackle a specific patient care problem and to understand his or her clinical thinking. It is a vehicle for previewing what a clinician intends to do in a partic­ular case.

Prospective proctoring can be particularly effective in the initial evaluation of a new medical staff or group practice member. It can occur quickly and be completed before a potentially problematic practitioner has cared for too many patients. The new practitioner is immediately drawn into a collegial relationship and, done well, the activity fosters a supportive—rather than threatening—atmosphere. For this reason, prospective proctoring is also useful if a goal is to add some element of mentoring into the competency assessment process.
